Under no circumstance shall work be undertaken in the proximity of such cables without the prior consent of WPD.Prior to the excavation operation progressing Utilities Plans must be readily available & consulted to determine the risk afforded by foreign services. By reference to the plans the ganger sweeps the area where the trench is to be dug using a CAT (Cable Avoidance Tool). Where live cables, BT lines or gas mains are detected he shall mark the surface by overspraying with aerosol paint i.e. the area within the painted lines is deemed the danger area. He then issues a Consent to Dig and instructs the men as to any requirements or restrictions to work which he may chose to impose.If foreign services are detected or their presence is suspected then trial holes must be opened, initially, by hand digging EXCEPT where they encroach into the footpath or carriageway when the surface may be broken using a road-breaker, stihl or road saw. If an abrasive tool is chosen then the surface to be cut must be continuously wetted to control the dust hazard. Only competent persons are to change discs on stihl & road saws. Whip-lash arrestors are to be used across all joints on compressed air baggings. If a power cable has been detected during the course of the initial sweep then the exposed area in the trial hole(s) must be swept at periodic intervals with the CAT. Forks, picks & other sharp pointed tools are not to be used when hand digging. Blunt ended wrecking bars (ringers) may be used to loosen large boulders etc. Where the plans illustrate that a gas main is present the use of the trencher within 500mm of the suspected lie is strictly prohibited UNTIL such time as it has been fully exposed by hand digging. Once the gas main has been fully exposed the trencher may approach to the 500mm limit PROVIDED THAT the trencher works away from the service. N.B. if mass concrete is exposed STOP WORK FORTHWITH and seek advice from your ganger. Do not restart work without his express consent.Traffic management in the form of two way traffic lights will be set up in accordance with Chapter 8. All signage and barriers will be checked prior to operations commencing. Barriers will be erected within the coned off area to add a further guard against motorists. With all power cables & gas mains fully exposed the trench may now be opened with the rock trencher. Before commencing the trencher a banksman must be present at all times, firstly to check depth of digging chain and to ensure that all workforce are working a safe distance from the trencher (the banksman shall make all operatives aware that nobody is to move behind the trencher and the lorry drivers must be briefed not to get out of their wagons). The chain will be carefully lowered into the ground and the conveyors will be started at the required speed. The chain will be lowered to the required depth. All operations will cease and the profile/depth of the excavation will be checked to ensure compliance with the specification. Once this has been satisfied the lorry will be positioned under the conveyor and the trenching continued. The speed of the conveyor shall ensure that the excavated spoil is carried to the lorry satisfactorily. Should there be a need to check the machine then the engine will be stopped. A warning will be displayed on the cab warning others not to start up the machine. As soon as the lorry is full he will pull out of the working area and adjoin the live traffic and remove the spoil to a licensed tip. A banksman will attend the lorry and trencher at all times. A second lorry will the position itself under the trenchers conveyor and the process repeated. All manoeuvres within the working area will be supervised by a qualified banksman. Whilst the trencher is in progress the banksman will monitor ground conditions & be continuously on the look out for unsuspected foreign services. If any such services are observed they shall STOP WORK FORTHWITH and seek the advice of their foreman.With the appropriate section of trench complete the pipe work is installed (see the Bolton Hill main method statement for this procedure) and the backfilling operation commences. This will be done by two 6T swivel dumpers that have been modified to make a funnel on the front of the skip, this enables the dumpers to straddle the trench and discharge into the trench without any overspill and negates the need of the dumpers having to get a wheel on the verge to discharge from the side. A Banksman must supervise this operation at all times making sure the trench is straddled centrally so as to avoid any breaking back of the edges of the tar and creating an unsafe condition on the excavation sides. All compaction equipment must be moved mechanically. N.B.if any man has to enter the excavation for any purpose the foreman shall first undertake a visual inspection before authoring such entry. If the foreman is of the opinion that such area of the trench is unstable or the trench is over 1.2 deep from bottom of blacktop to the top of the pipe bedding he shall determine the nature of any temporary support (shoring), arrange for it to be delivered to site & supervise its installation. Persons shall not enter the trench until such support has been afforded. SUPPORTING INFORMATION PROJECT EMERGENCY PROCEDURES 1.0 INTRODUCTION The purpose of developing an Emergency Action Plan is to focus the Project Management Team on their priorities in the event of a real emergency. Their prime objectives are to: Save life Relieve suffering Minimise loss Contain the incident Resume normal working as soon as possible. These procedures describe the planned response to such an event. EMERGENCY RESPONSE 2.1 CABLE STRIKE 2.1.1 Workmen are not in the trench The machine operator shall a) Stay in the cab of his machine until he is informed, by the statutory undertaker, that the cable is de-energised and that it is safe to do so OR b) His life is threatened by another hazard, e.g. fire, in which case he is to jump clear from the cab. THEY MUST NOT CLIMB DOWN HOLDING ANY PART OF IT OR THEY WILL BECOME THE EARTH PATH AND BE ELECTROCUTED. The banksman shall a) Remain on guard in the hazard area, until relieved by his supervisor, to prevent others from touching the machine or entering the trench; and b) Immediately make contact with his supervisor by telephone or radio to appraise him of the incident. Where the supervisor is the senior person in charge on the site, he shall a) Promptly inform the statutory undertaker in order to organise the isolation of the power supply to the affected cable; and b) Thereafter inform the Project Manager. Where the supervisor is not the senior person in charge on the site, he shall a) Promptly inform the senior person in charge on site and b) Thereafter make his way to the hazard area and relieve the banksman. The senior person in charge on site shall a) Promptly inform the statutory undertaker in order to organise the isolation of the power supply to the affected cable b) Where the incident is deemed reportable under the provisions of RIDDOR, thereafter inform those identified in the Emergency Plan (S11 H&S Plan) 2.1.2 Workmen are in the trench The procedure described in 2.1.1 above is followed with these additional provisions. The workmen shall Immediately evacuate the trench leaving any casualty behind. Where a casualty has been identified The supervisor shall Summons the Emergency Services by dialling 999 and informing the operator of The nature of the incident; The precise location; The number of casualties. b) Then follow the procedure identified in S 2.1.1 above. 3.0 GAS MAIN STRIKE 3.1 The workmen shall Cease operations forthwith; Switch off any power tools and mechanical excavators; Prohibit smoking; Evacuate the excavation and post sentries to prohibit entry into the danger zone; Promptly inform the foreman. 3.2 The Foreman shall Promptly inform the local project office to appraise them of the situation. The Local Office shall Promptly contact the statutory undertaker to arrange any repair EXCAVATION COLLAPSE 4.1 Men are not in the excavation 4.1.1 The foreman / ganger shall Forthwith instruct his team not to enter the excavation; Inform the Agent by telephone; Post a sentry to prevent others entering the danger zone. 4.1.2 The Agent shall In consultation with the foreman / ganger make an assessment of the condition of the excavation and determine the safe method of recovering the situation; Inform the Project Manager; Complete a PAIR Report (the incident is to be treated as a close miss). 4.2 Men are in the excavation Nobody is trapped or injured Follow the procedure outlined in 4.1 above. 4.2.2 A workman is trapped or injured; REMEMBER THE PRIORITY IS TO SAVE LIFE DO NOT PUT YOUR OWN LIFE AT RISK BY ENTERING A COLLAPSED EXCAVATION WITHOUT FIRST ASSESSING THE RISK OF FURTHER COLLAPSE AND, WHERE NECESSARY, INSTALLING TEMPORARY SUPPORT. The foreman / ganger shall Immediately summons the Emergency Services by dialling 999; Inform the Agent; Make initial preparations to recover the trapped or injured. The Agent shall Forthwith inform the Contract Manager and Immediately make his way to the incident to assume control. The Contract Manager shall Forthwith inform those identified in the Emergency Plan (S11 H&S Plan). 5. FIRE Only persons who have been trained in the use of portable extinguishers are to attempt to fight fires. In the event of a fire: The Workmen shall Evacuate the immediate danger zone and post sentries to prohibit others from entering (particularly members of the general public); Inform their local fire warden; Where there is no local fire warden, immediately summons the Emergency Services by dialling 999, giving them the following information: Locality and nature of the incident; Numbers of casualties (if any).
